In addition, please be prepared to upload a copy of a resume that includes any leadership and involvement experiences you have had in high school, college, or graduate school, during employment, with community organizations, in the military, or in other settings.

Finally, a letter of recommendation from an individual familiar with your leadership experience and potential should be submitted by the recommender directly, via the separate online Recommendation Letter Submission Form.

I understand that I must meet certain qualifications to be eligible for admission to the Dental Student Leadership Institute and continuation in the Institute, including that I: must be in good academic standing; have no course failures or remediation; cannot be on Monitored Status at the time of the experience, and have not been found responsible for violations of the NYU Dentistry Code of Ethics and Professional Conduct; and availability for special DDSLI activities and events including summer retreats and workshops, internships, off-campus interviews or meetings with oral health leaders, and project planning activities with student peers.
